Expert Recommendations for Heart Patients Who Wish to Fast
Individual assessment is highly important for heart patients who want to fast during Ramadan. Experts emphasize that cardiovascular diseases can progress differently in each patient and stress that a medical evaluation and personalized risk assessment should be completed before deciding to fast.

Medication Schedule Should Not Be Disrupted
A significant proportion of heart patients use medication regularly. According to experts, medication times can be reorganized according to the fasting schedule, but this adjustment must be made under medical supervision. Skipping medication, reducing the dose, or making changes independently may lead to serious health problems. Patients with hypertension, arrhythmia, and heart failure in particular need to be careful when planning their medication schedule.
Fluid Balance Must Be Maintained
Prolonged dehydration may cause low blood pressure, palpitations, or rhythm problems in some heart patients. In addition, insufficient fluid intake can reduce blood flow fluidity and increase the risk of clot formation. For this reason, adequate water should be consumed between iftar and sahur, and beverages that may increase fluid loss such as excessive tea and coffee should be avoided. Patients who have previously experienced vascular blockage or have a history of clotting need to be especially cautious.
Avoid Sudden Overeating at Iftar
Eating quickly and excessively after prolonged fasting may increase the workload on the heart. Balanced, light, and portion controlled meals should be preferred instead of fatty, salty, and heavy foods. Starting the meal with light options such as soup and transitioning slowly to the main course is recommended. Sudden calorie loading may negatively affect blood pressure and heart rhythm.
Which Symptoms Should Be Taken Seriously?
Some complaints that occur during or after fasting should be taken seriously:
- Chest pain
- Shortness of breath
- Palpitations
- Dizziness
- Feeling faint
If any of these symptoms occur, medical attention should be sought without delay.
Conscious Planning Is Essential
Acting consciously is essential to protect both health and religious practice during Ramadan. For heart patients, the most appropriate approach is to create a plan tailored to individual health conditions and manage the process in line with medical advice.
